In other words, at the group home for elderly people with dementia in downtown Tokyo where I worked, there was a strong emphasis on supporting the residents’ autonomy and independence. Staff members were expected to stay in the background, like supporting actors in a puppet theater performance. Whether it is cleaning their rooms, preparing meals, or shopping for ingredients, if residents with dementia express a desire to do these tasks, the staff will support them. Even if they don't, the staff will offer options to encourage their participation, allowing the residents to take the initiative as much as possible. The staff support their daily lives from behind the scenes, much like backstage crew in a performance.
I remember that at the group home for elderly people with dementia in downtown Tokyo where I worked, there were usually at least three staff members on duty during the day, working early, daytime, and late shifts. Almost every afternoon after lunch, one staff member would go grocery shopping with two or three residents at a nearby supermarket. Since there were still multiple staff members remaining in the unit, these kinds of activities were possible. I think the favorable staffing situation may have been due to the fact that the facility was operated by a social welfare corporation. As a work environment for elderly care, it was quite supportive. However, it has been nearly ten years since I worked there, so I am not sure what the situation is like now.

つまり、私が勤務していた都心にあった認知症対応型グループホームでは、認知症である利用者の自主性や自立性が重視され、あくまでも職員は影の存在であり、いわば人形浄瑠璃の黒子のように振る舞うことが求められていたのだろうと思います。居室の掃除にしても、自分たちが食べる食事の準備にしても、あるいは食材の調達にしても、利用者の方から希望が出されればそれに従いますし、出されなくても、職員が選択肢を提示したりすることによって、基本的には入居している認知症の利用者たちが主体的に取り組むことになります。職員は、いわば陰のサポート役として利用者の日常生活を支えるわけです。
私が勤務していた、都心にあった認知症対応型グループホームでは、日中の時間帯は、職員が早番、日勤、遅番と、ほぼ常に3人以上でシフトが組まれていたと記憶しています。ほぼ毎日のように、昼食後、職員1人が2～3人の利用者とともに、近所のスーパーまで食材の調達のために買い物に行っていましたが、まだユニット（グループホームの一つの単位）には複数の職員がいたので、こういった様々な活動も可能でした。社会福祉法人が運営していたということもあったのではないかと思いますが、でもこの職員のシフトの状況は、介護を行う環境としてはかなり恵まれていたと思います。ただ、私が勤務していた時から10年近くの時間が流れてしまっているので、今はどうなっているのかちょっとわかりません。
