While I was working at the group home for people with dementia, I also experienced end-of-life care issues involving both my grandmother—who had dementia and was living in a nursing home—and a resident at the group home. These experiences made me reflect deeply. For instance, at the very moment a terminally ill resident was suffering in her room, a Christmas party was taking place in the main hall, with other residents and staff celebrating there. I still do not know what I should have done, and I have not found an answer to this issue. Therefore, I have no intention of blaming anyone or asking who was right or wrong, or what should have been done differently. I simply find myself questioning the reality of what happened. It seemed that even the family members who came to care for the suffering resident did not fully grasp the situation. I could not help but feel that they did not truly understand or envision what end-of-life care entails, and were merely following the instructions given by the facility and the care manager.
Care workers who interact daily with elderly clients—many of whom may pass away at any time—should never become desensitized to death, and the dignity of every elderly person in care must be protected. As population aging continues to accelerate, such situations will become increasingly common in the field of care. For the sake of this country’s future, I believe we must reexamine our ethical approach to life and death.

また、この認知症対応型グループホームで勤務していたときに、有料老人ホームに入所していた認知症の祖母とグループホームの利用者の、終末期の看取りの問題に接する機会があり、とても考えさせられました。例えば、終末期にあったグループホームの利用者が居室で苦しんでいるまさにその時、ユニットの大広間では他の利用者たちや職員たちがクリスマス会を行っているといったことがありました。私自身、どうしたら良かったかわかりませんし、どうすべきだったか、今だに答えは見つかっていません。なので、べつに誰が悪い、どうすべきだったか、と誰かを責めるつもりは全くないのですが、ただこの現実はいったい何なのだろうかと思ってしまいます。介護に来ていたその苦しんでいる利用者の家族も、その状況を把握できていなかったのではないかと思います。実際に看取りとはどういうことなのかを理解しイメージすることができておらず、施設やケアマネの言うとおりにしていただけだったのではないのか、そんなふうに感じざるを得ませんでした。
いつ亡くなってもおかしくはない高齢の利用者の方々と日々接している介護職員であればこそ、利用者が亡くなっていくことに慣れてはいけないでしょうし、利用者の方々の尊厳は守られなければならないと思います。高齢化率がさらに上昇していく近い将来、介護の現場では、さらにこのような機会が増えていくのではないかと思いますが、人間の生死に関わる倫理観を、この国の将来のために、改めて問い直さなければならないのではないかと思いました。
