Tasks at the group home for dementia care D
As I have mentioned, although it was a group home, it differed significantly from others, largely because the parent organization operating the care service was different. Naturally, this led to a different philosophy of care. For reference, I have summarized below the tasks I performed during my night shifts as a part-time care worker at this group home. Please note that conditions—such as the number of staff and working hours—can vary depending on the facility, so this should be considered as a general reference only.
16:50 – Handover: Share important information, etc.
17:00 – Replace evening medicine box.
– Serve roasted green tea.
– Replace plates displaying the schedule for the following day.
– Update magnetic plates showing the date, day of the week, and names of shift staff. Check the day’s schedule on the calendar, etc.
17:30 – Hand disinfection.
17:45 – Dinner.
18:15 – Oral care and assistance with going to bed.
19:15 – Cleaning:
• Toilets – Put germicidal bleach into the green bucket and use a toilet mop.
• Sinks – Clean with sponge and abrasive cleaner.
• Toilet bowls – Wipe around the rim with a sponge, clean the seat with toilet cleaner, then scrub the inside with a toilet brush. Repeat the same cleaning steps for the bathroom toilet and the two toilet bowls in the laundry room.
19:30 – Prepare the kitchen.
– Set out towels, two dish towels, and two table towels.
– Fill the kettle with water.
– Check the fire extinguisher.
19:45 – Check medication items.
– Confirm the resident name, date (for the following day), and distinguish between morning, afternoon, and night doses.
20:00 – Prepare oral care items for after breakfast.
Hairbrush, razor, cup, toothbrush, and dentures.
• Conduct rounds at 20:00, 22:00, 00:00, 03:00, and 05:00.
During each round of checks, record observations as needed.
• At 00:00, summarize records for the day.
– Record the total amount of fluid intake, and the number of urinations and bowel movements in the designated log.
22:00 – Dispose of used medicine bags.
• Use the bathroom toilet during the night if needed.
00:00 – Count and record the number of times each resident used the toilet.
– Count and record each resident’s total fluid intake.
– Update the date on the daily record. Write the resident’s name and the new date on a fresh sheet or the reverse side of the current one.
– On the vital signs and excretion checklist, record the number of days since the last bowel movement.
05:00 – Open the curtains.
– Turn on the TV.
– Warm up some tea.
– Collect trash from the toilets.
– Clean dentures.
– Prepare the bathroom.
06:00 – Call out to residents to wake them up.
06:30 – Check vital signs.
07:00 – Radio exercises.
07:30 – Hand disinfection.
07:45 – Breakfast.
08:30 – Style the hair of the resident who has just finished bathing.
09:00 – Refill excretion pads in the toilets.
09:30 – Record notes and observations.
The duties listed here are general in nature. Therefore, I have intentionally omitted specific details—such as individual assistance with excretion or bedtime care—in order to protect personal information. Naturally, there were times when we had to respond to residents who woke during the night or visit their rooms individually to assist with excretion, so please consider the duties described here to represent only the minimum required tasks.

認知症対応型グループホームDでの業務
これまで述べてきたように、グループホームとは言っても、他のグループホームとは大きく異なっていましたし、介護事業を行っていた運営の母体も異なっていたので、当然介護に対する理念も異なっていたことだろうと思いますが、一応参考のために、このグループホームで、私がパート職員として夜勤をしていた時に、どのようなことを行っていたのかについて、以下にまとめておきました。職員の数ですとか、勤務時間など、介護事業所で条件は様々に異なるものだろうと思いますので、あくまでも参考程度のものとお考えください。
16 : 50 申し送り。注意事項など。
17 : 00 夜用の薬箱に交換。
ほうじ茶を配茶。
明日の予定のプレートを交換する。
日付、曜日、シフト職員の名前。その日の予定をカレンダーなどで確認する。
17 : 30 手の消毒。
17 : 45 夕食。
18 : 15 口腔ケア、就寝介助。
19 : 15 掃除
トイレ グリーンのバケツに殺菌漂白剤を入れて、トイレ用のモップを使う。
洗面台 スポンジと研磨剤で。
便器 スポンジで回りを拭いた後に、クリーナーで便座を拭く。そのあとブラシでこする。
風呂場のトイレと洗濯室の２つの便器も同様に。
19 : 30 台所の準備
タオルのセット、フキン２枚、台フキン２枚のセット。
ポットのお湯を足す。
消火器の確認。
19 : 45 薬の項目のチェック。
名前、日付（次の日）、朝昼夜の区別。
20 : 00 朝食後の口腔ケアの準備。
ブラシ、髭剃り、コップと歯磨き、入れ歯。
・巡視は、20 : 00、22 : 00、0 : 00、3 : 00、5 : 00に行う。
見回りおよび記録
・トータルの水分量、排尿・排便の回数は、0 : 00で閉めて、所定の所に記録する。
22 : 00 薬袋を廃棄する。
・夜間のトイレは風呂場のトイレを使う。
0 : 00 トイレ数を集計して記録する。
水分量を集計して記録する。
生活記録の日付を変える。新しい面もしくは新しい用紙へ名前と日付を記入する。
バイタル、排泄チェック表の排便のマイナス何日目を記入する。
5 : 00 カーテンを開ける。
テレビをつける。
紅茶をあっためる。
トイレのごみを集める。
入れ歯を洗浄しておく。
風呂場の準備。
6 : 00 みんなを起こす。声掛け。
6 : 30 バイタル測定。
7 : 00 ラジオ体操。
7 : 30 手の消毒
7 : 45 朝食
8 : 30 風呂から上がってきた人の整髪。
9 : 00 トイレのパッドの補充。
9 : 30 記録
ここに記載している業務は、あくまでも一般的な業務なので、個々の利用者に対する排泄の介助ですとか、就寝時のケアなどについては、個人情報の保護の観点から、記載することを控えさせていただいています。当然のことながら、利用者の方が夜間に起き出してきて対応したり、排泄介助のために個別に居室に伺ったりすることもあったので、ここに記載していることは、あくまでもミニマムな業務だとお考えいただければと思います。
