What is a care manager?
In Japan, a specialized position called a long-term care support specialist—commonly known as a care manager (care-mane)—has been established to coordinate and manage elderly care and social services for older adults in need of care. Under the Long-Term Care Insurance Act, a care manager is defined as a professional who responds to consultations from people in need of care, coordinates with municipalities and care service providers, etc., and has been certified as possessing the specialized knowledge and skills required to provide the assistance necessary for clients to live independently. In other words, the primary role of care managers in the long-term care insurance system is to assess the care needs and health conditions of elderly clients and their families, develop individualized care plans, select and coordinate appropriate care service providers, and regularly monitor and evaluate both the clients’ health status and the appropriateness of the care and services being provided.

介護支援専門員
介護支援専門員とは
日本では、介護を必要としている高齢者のために、介護サービスや社会的サービスの調整、管理を司る“介護支援専門員”（ケアマネジャー、ケアマネ）と呼ばれる専門職が導入されています。ケアマネは、介護保険法において、要介護者等からの相談に応じ、市町村や介護サービス事業者等との連絡調整等を行うものであり、利用者が自立した生活を営むように必要な援助に関する専門的な知識や技術を有するものとして介護支援専門員証の交付を受けたもの、などとして定義されています。言い換えると、ケアマネが介護保険制度において果たす主要な役割は、高齢者とその家族のケアニーズと健康問題を評価し、個々の利用者についてのケアプランを作成するとともに、ケアサポートサービス提供者を調整し、そして定期的に利用者の健康状態と、ケアサポートやサービスの適切さをモニタリングして評価することになります。
