Now, this is only speculation, but had the head of the facility or the senior care manager told me that the client would be sent back to her original long-term care health facility—despite the family’s consent and the client’s strong wish to return home—I would certainly have opposed it. In fact, if she had made proper use of welfare equipment and utilized outpatient and accommodation services effectively, I believe it would have been entirely possible for her to live at home. Above all, she was highly motivated. Therefore, I felt it was my duty as a care manager to support her in achieving that. When I considered why she had to return to her original facility despite this, I could not help but think that, first of all, it may have been due to the convenience of her family as caregivers. More importantly, however, it may have been a case of something like intentional social hospitalization by care facilities—locking in users in ways that probably managed to avoid violating the law. In any case, if a care manager cannot fulfill the strong wishes of the users, then such care managers have no reason to exist, and I felt there might be no need for such a role. In the end, I left this small-scale multi-functional care facility after only a short time, but I believe that was when my decision to resign was truly solidified.
As I mentioned earlier, small-scale multi-functional care facilities provide both day care and visiting services, which require making local rounds by car. Because I do not have a driver's license, I had to rely on accompanying a senior care manager or facility nurses. This restriction limited my role as a care manager and was one of the reasons I decided to leave.

ここからは推測なのですが、ご家族からの了承も得られて、しかもご本人が自宅に戻る気満々でいるなかで、もし私に、この利用者がもとの介護老人保健施設に戻ることになったと施設長か先輩ケアマネの方から伝えてきたら、確かに、私は強く反対しただろうと思います。実際、福祉用具をうまく使い、通いや宿泊サービスを有効に使えば、自宅での生活は十分に可能だっただろうと思いますし、何よりも本人がやる気満々だったわけですから、ケアマネとしてはそれを実現できるようにサポートするのは義務だろうと思います。それがわかっていて、なぜ介護老人保健施設に戻らなければならなかったのかを考えた場合、まず介護者であるご家族の都合があったのかもしれませんが、何よりも、施設による利用者の囲い込みのような、おそらく法に触れることはないものの、実質的に介護施設による意図的な社会的入院に類するようなことがあったのかもしれないと考えざるを得ませんでした。とにかく、利用者が強い希望を持っているにも関わらず、それをケアマネとして叶えることができないならば、そんなケアマネに存在意義はあるはずはないでしょうし、そんなケアマネならいなくてもいいのではないかと思いました。結局この小規模多機能型居宅介護事業所も短期間で辞めることになりますが、辞める意思はこのときに固まったと思います。
前にも述べましたが、小規模多機能型居宅介護事業所というのは、訪問サービスや通所サービスがあるため、近隣地域を車で巡回する必要があるのですが、私には自動車の運転免許がないため、地域を巡回するにしても、先輩のケアマネや看護師などに連れて行ってもらわなければならず、小規模多機能型事業所でのケアマネとしての活動に限界を感じていたことも、辞める理由の一つとなりました。
