Although dementia is already a major concern and is projected to become even more serious in the near future, analyzing it in the context of environments to which humans have adapted over evolutionary timescales may yield insights that help improve elderly care. For example, many hypotheses have been put forward regarding the possible influence of natural selection—one of the primary forces driving evolutionary change—on Alzheimer’s disease. The idea that Alzheimer’s disease is a ‘disease of civilization’ has also been suggested. I anticipate that the emerging field of evolutionary medicine—which investigates human physical and mental conditions, including health and disease, from an evolutionary standpoint—will yield valuable insights into the complex realities of caregiving, which appear to involve many real difficulties. A deeper understanding of the innate human tendencies shaped over evolutionary time—such as those underlying parenthood and grandparenthood—may guide us toward more natural and less stressful approaches to elderly care. Such approaches could promote more rational, home-based caregiving and reduce burdens not only on elderly clients but also on the staff who support them in care facilities. Consequently, I am convinced that adopting a multifaceted perspective on our current, highly challenging situation is crucial for moving forward.

認知症は現在、深刻な問題であり、将来さらに深刻になると予測されていますが、このように、人間がこれまでの長い進化の過程で適応されてきた“進化的に適応されてきた環境”という観点から認知症のことを考えてみることは、将来におけるより良い介護につながる手がかりを与えてくれるのではないかと私は考えています。例えば、自然選択は進化的な変化をひきおこす主要な要因ですが、認知症をひきおこす主要な原因疾患であるアルツハイマー病に、その自然選択がどのように影響を与えうるかに関して、これまで様々な理論が提起されており、アルツハイマー病が“文明病”である可能性も提起されています。人間の病気や健康といった身体や精神の状態を、進化の観点に照らして考えていく進化医学という新しい領域がありますが、この進化医学からの知見は、これまで長々と述べてきたように、実際に多くの困難を抱えてしまっているようにみえる介護の現実に対して、多くの役に立つ有用な洞察を与えてくれるのではないかと私自身とても期待しています。例えば、親としての性質や祖父母としての性質といったものを、長い長い進化の過程で人間が獲得してきた自然本性として理解することができれば、実際に家庭内で介護をするときにも、より合理的な理解のうえで介護を実践することができるでしょうし、介護施設で介護職として介護するときにも、高齢利用者だけでなく介護職員にとってもストレスのあまりかからない、より自然な介護の実践にもつながるのではないかと思います。現在、多くの困難に満ちているようにみえる介護の実際の状況に関する様々な視点は、将来のためにも不可欠なものであると私は強く考えています。
